Sheet Harbour Family Practice Survey

The Sheet Harbour Family Practice currently sees many patients and community members having to travel to other areas of the province to access certain care. This survey will help us assess what care those folks are traveling to access, with the goal of finding gaps that we can fill within the Practice. Filling these gaps here in the community will mean patients can avoid unnecessary travel and access the care they need closer to home.

  

The purpose of this survey is to identify care gaps in Sheet Harbour and have a good picture of community needs. Although the Sheet Harbour Family Practice does not presently have resources to implement new services, we want to identify needs to help us expand our services in the future. 


This survey will take you approximately 5-10 minutes to complete. Your responses are anonymous, and your participation is greatly appreciated.
