Why are we doing this survey?

    Public Health is collecting this information to make sure we have accurate information about vaccine coverage for the population. Your child’s information will be used to do a quality check with our database and for counting (statistical) purposes. 

    What will you do with the information?

    We will analyze the information collected to understand our coverage rates for immunization in children.  The survey will also provide us with information about how to improve immunization services in Nova Scotia.  We will create a report that will be shared within Nova Scotia Health Authority and Department of Health and Wellness. We will also create a summary report that will be shared you and others who are interested in the survey results. 

    What will happen after the survey is finished?

    The survey is part of a two year project.  The survey results will help with the second part of the project.  Based on what we hear from parents/guardians in the survey, a plan will be developed to address barriers that parents reported experiencing.  We will focus the interventions in the communities that are facing the most challenges related to immunization. 

    What do I do if I don’t want to participate in the survey?

    All points of view about immunization are important and welcome. However, if you do not want to take the survey, please call 1-844-996-0694 or email ImmunizationSurvey@nshealth.ca and leave a message with your child’s name and date of birth. Your decision will not affect your child’s health record and future care.

    What could you win for helping?

    We appreciate the time it takes to complete the survey and know parents have very busy lives.  We appreciate that you took the time to share the information we are looking for. 

    As a thank you for completing the survey, we will be drawing 14 names to win a prize.

    Winners can choose between a:

    • $50 grocery store gift certificate from the store of your choice

    What happens if I haven’t immunized or don’t have my immunization records for my child?

    We are seeking all points of view about immunization.  We are looking for your perspective.  If you don’t have your child’s immunization record, we will get it for you.  You can still complete the survey. 

    Will you keep my information confidential?

    Yes.  We keep all health information collected as private and confidential according to the policies of Nova Scotia Health Authority.  We will retain the information according to our records retention policies. 

    Will you be sharing our information with others?

    No.  Once the quality check with our immunization records is complete, all the other information collected will be kept anonymous and counted for statistical and planning purposes. We will not share the personal information you give us with anyone else and we will make sure it is kept safe. 

    Where does the funding come from to do this project?

    This project was funded by the Immunization Partnership Fund of Canada.  NSHA Public Health would like to take this opportunity to recognize and thank Public Health Agency of Canada for this opportunity.  

    When will I receive my child's immunization record I requested as part of the survey?

    Due to the large volume of requests for immunization records, it takes 3 - 4 weeks for immunization records to be sent back to parents.  If there are any data quality issues, it does take longer than this time frame and for some parents, a Public Health Nurse may call them to discuss their child's record.  We will be continuing to ensure you get your child's information and should have all the records returned to parent by the end of June.  We really appreciate your patience.
