Public Health Mobile Unit Post-Immunization Client Survey

Our Public Health Mobile Unit offers optional feedback surveys to clients during their post-vaccination recovery period. Clients complete paper surveys with staff support available as needed. Survey results are tabulated monthly and annually and are used to inform quality improvement efforts and program planning. We've revised our survey and are looking for feedback on the new version. This feedback will help us finalize the new Post-Immunization Client Survey to better support quality improvement and program planning.
