Background Information

The Care Transitions: ICU to Step-Down Care one pager is being developed after a recent Quality Review at Valley Regional Hospital identified the opportunity to improve communication and expectation setting for patients and families as they leave the Intensive Care Unit (ICU) and transition to step-down units.
