Background Information

We are planning to update how we report information from the Need a Family Practice Registry each month so that it is easier to understand and gives a clearer picture of why people sign up and how they move off the registry. We are looking for feedback on a visual of what we are considering the new report to look like.

In the email request, there are two documents: 

  • Document A: Provides some background information on the project.
  • Document B: Shows a visual of what we are considering the report to look like- This is the document we are looking for feedback on!


Note: When we say “Resolved” we mean a person no longer requires support to find a family doctor or nurse practitioner (for example, they have moved, are deceased, found a family doctor or nurse practitioner on their own, etc).
