About the Diabetes in Pregnancy Model of Care Project: The Diabetes in Pregnancy Model of Care Project involves a group of care providers from across Nova Scotia (NS) who are examining diabetes care for the pregnant population including existing resources and service delivery. The working group is exploring the development of a standardized, evidence-based diabetes in pregnancy model of care for NS. The purpose of the patient survey is to assist with better understanding the function and current state for persons receiving diabetes care during pregnancy.

    We are reviewing the ways diabetes care for pregnant persons is delivered in Nova Scotia. What we hear will be used in creating a model for diabetes care while pregnant that works for Nova Scotians.

    • Understand current services, resources and gaps in diabetes care during pregnancy.
    • Identify opportunities to improve and simplify patients’ journey from referral to treatment.
    • Improve diabetes care in Nova Scotia.
